print out the Noninstructional District Employment Application
and accompanying forms. There are nine steps to ensure your application
is complete.

District Employment Application
Item 2: Print out and complete
the top portion of the Employment
Eligibility Verification Form I-9
Item 3: Print out and have your
physician complete the School
Personnel Health Record.
(Note: Tuberculosis Test must be
applied within the last 3 months)
Item 4: Print out, complete,
and forward with enclosed $10.00 Money Order
only the Pennsylvania
Child Abuse History Clearance.
Item 5: Print out, complete,
and forward with enclosed $10.00 Certified Check/Money
Order only the Request
for Criminal Record Check
OR
Apply online with a required credit card at Pennsylvania
Access to Criminal History
Item 6: Current Act 114 (Federal
Criminal History - Apply Online)
Item 7: Print out and complete
the bottom portion of the Employee’s
Withholding Allowance Certificate Form W-4
Item 8: Print out, sign and
date the Notice
to Employees of Bethlehem Area School District in Case of Work-Related
Injury
Item 9: Post recommendation
drug testing.
